YOUR SELF-OBSERVATION RECORD
For the next several days, record each time the target SDB occurs. Make a mark for each
time that you experienced your SDB. Then, fill in the other items designated: conditions noted, other people involved, thoughts,
feelings, etc. After several days, review what you have recorded. You will have a much clearer picture of what is involved
in your SDB. This is a required activity and is graded as complete/not complete. Copy and
paste this assignment onto a Word document before you complete it. Remember to
use the “Track Changes” tool.

What is the SDB?
What are the conditions you are in when you do the SDB?
Who are the other persons involved, if any?
What are your thoughts about this?
What are your feelings about this?
What actions did you take?
When you have entered your data for the entire week or more, go back and review the information
that you have collected about how your Self-Defeating Behavior impacts your life. Armed with this information, you will be
in a better position to make changes.
